In your e-mail, please include your treatment diagnosis or main complaint, primary and secondary insurance information (id number, expiration date, 800 number) and a contact phone number to reach you. Our reception staff will contact you within normal business hours with an appointment time.

If you are a new physical therapy patient, please download the “New Patient” forms and print them to fill them out before coming in for your appointment. Also, please download the “Privacy Practices” document and review it.

If you are seeing us for treatment of urinary incontinence, please download the “Continence Questionnaire” and complete before your first visit; in addition, please download the “Bladder Diary” and complete 3 days of tracking your bladder habits before your first appointment.
