If you are a new physical therapy patient, please download the "New Patient" forms and print them to fill them out before coming in for your appointment. Also, please download the "Privacy Practices" document and review it.
If you are seeing us for treatment of urinary incontinence, please download the "Continence Questionnaire" and complete before your first visit; in addition, please download the "Bladder Diary" and complete 3 days of tracking your bladder habits before your first appointment.
If you or your child will be seeing Andrew Gardner, PhD, for behavioral analysis, please download and complete all of the forms below. You will need to bring these with you to the first appointment. Allow plenty of time to complete the forms well in advance of your appointment.
